IssueDetail
Issue Detail: The Sacramento Fire Department (SFD) is seeking approval to establish a multi-year operating project for the department’s Health and Wellness Program. Currently, many health conditions are identified only after symptoms become severe enough to require medical intervention, resulting in increased treatment costs, lost work time, disability leave, and operational impacts. Early detection, preventive care, and ongoing health monitoring can identify health concerns before they progress into more serious and costly conditions, improving employee outcomes while reducing the financial impact of presumptive occupational disease claims.
The Health and Wellness Initiative identified proactive medical and physical health screenings as a critical component of a comprehensive strategy to improve employee health, reduce workers’ compensation costs, and maintain workforce readiness.
The previously approved annual allocation of $350,000 from the Public Provider Ground Emergency Medical Transportation (PP-GEMT) Fund will provide a sustainable funding source for comprehensive medical and physical health screening programs. These services may include cancer screening, cardiovascular assessments, laboratory testing, fitness and movement evaluations, injury risk assessments, and other evidence-based wellness services designed to identify health concerns early and support preventative care.

PolicyConsiderations
Policy Considerations: The proposed recommendation supports the City’s responsibility to provide a safe and effective workplace for employees while promoting fiscally responsible management of public resources. The recommendation is consistent with the City’s obligations under California workers’ compensation laws and occupational safety requirements to provide appropriate injury management and return-to-work practices for employees injured in the course and scope of employment.
The recommendation and implementation of a proactive wellness and injury prevention program support the City’s objectives of reducing long-term liabilities, reducing costs, improving employee retention, minimizing operational disruptions, and maintaining emergency response readiness for the community.

EnvironmentalConsiderations
Environmental Considerations: Ongoing administrative maintenance activities, such as purchases of supplies, equipment, or materials which are not made for purposes of a public construction project, do not constitute a “project” and are exempt from the California Environmental Quality Act (CEQA), CEQA Guidelines, Section 15061(b) (3), 15378(b) (2).

RationaleforRecommendation
Rationale for Recommendation: The establishment of the Health and Wellness Program is intended to directly address the increasing frequency and severity of firefighter injuries and the associated operational and financial impacts on the organization. By providing immediate access to injury assessment, preventative care, rehabilitation services, and return-to-work support, the position is designed to reduce the duration and severity of work-related injuries, improve employee health outcomes, and minimize time away from work. This program is expected to improve continuity of care, reduce delays in treatment, improve recovery outcomes, and reduce dependency on reactive injury management processes.
Research and comparative analysis of wellness programs implemented by other fire departments demonstrate that early intervention and department-specific rehabilitation programs can significantly reduce injury duration, expedite return-to-work timelines, and decrease overall claim costs.
End
FinancialConsiderations
Financial Considerations: This request authorizes the establishment of a multi-year operating project for the department’s Health and Wellness Program with ongoing allocation of up to $350,000 annually from the Public Provider Ground Emergency Medical Transportation (PP-GEMT) Fund to support proactive medical and physical health screening programs for Fire Department personnel.
